Case Manager

Case Manager - Full Time Permanent position

Salary – DOE

The Case Manager has twenty-four hour accountability through flexible work schedule. Maintains responsibility for coordinating hospital resources to provide pre-defined outcomes within a specified time frame for an assigned caseload. This position requires that the Case Manager is an active role model to Nursing staff for fostering critical thinking skills and other aspects of professionalism. The goal is to attain the highest level of practice from each direct care provider.

Meets internal and external customer service expectations and promotes the continuous improvement philosophy of the department and Hospital.

• Establishes and maintains effective working relationships with all customers, and resolves customer concerns in as prompt and effective a manner as possible.

• Participates in determining customer expectations through a complete assessment.

• Annually sets and achieves personal goals for improving customer service performance. Personal goals support departmental and hospital goals.

• Participates on inter- and intradepartmental continuous improvement teams.

• Demonstrates efficient and effective use of resources in meeting customer expectations and performing job responsibilities.

Provides leadership in the effective integration of system wide and patient care service initiatives into the delivery and management of patient care.

• In collaboration with the appropriate administrative and clinical staff, coordinates the activities of the multidisciplinary health care team.

• Identifies systems's problems and coordinates resolution utilizing multidisciplinary and administrative resources. • Participates in appropriate committees/task forces to facilitate implementation of case management and health care team programs and recommendations.

Serves as the primary coordinator of care for selected patients on an ongoing basis to identify, achieve, and evaluate desired patient outcomes, with consideration for resource management and patient needs throughout the continum of care.

• Participates in patient care rounds with the multidisciplinary team on a regular basis, with consideration for effective clinical/fiscal management of care.

• Serves as a consultant to outpatient and community agencies for the management of clinical problems and facilitation of patient care across the continuum. • Develops teaching protocols, materials and programs for patient/family/care provider.

Serves in a consultant role to staff, management, and health care professionals within the Hospital and the community in the areas of expertise. • Is readily available to staff to provide information regarding program planning, implementation, or evaluation and clinical consultation in areas of expertise. • Serves as a liaison to the community and other organizations for the case management program, health care team and as a clinical consultant in the area of expertise. • Provides formal and informal education to staff, community groups, or other health care professionals. • Involvement in professional organizations, publications, or other positive contributions to the nursing profession. Demonstrates the knowledge and skill necessary to direct care appropriate to the age and clinical needs of the patient served.

• Demonstrates the ability to evaluate data reflective of the patient's status and interprets the information to identify each patient's requirements relative to age-specific needs.

• Demonstrates knowledge of the principles of growth and development over the life span (appropriate to practice area).

• Maintains adequate documentation of the discharge plan on the patient record.

Educational / Minimum Requirements:

Bachelor’s degree in Nursing preferred.

Four to five years of recent experience in medical/surgical and/or critical care nursing.

State/Federal Mandated Licensure or Certification Requirements:

Current state of Connecticut RN Licensure
